Step One: Complete The New Patient Questionnaire Form
As part of our New Patient Enrollment Process, we kindly ask you to complete our New Patient Questionnaire Form, which helps us understand your unique needs. This form is the first step in getting to know you and tailoring our care to meet your goals.
Step Two: Medical Records
Please forward all medical history to info@luminapsychiatry.org. If there is no medical history please send us an email
Step Three: Scheduling Complimentary 10 minute Consultation
After initiating the New Patient Enrollment Process, if approved you will receive an email from info@luminapsychiatry.org. This email contains a secure link to access our calendar in order to schedule your complimentary 10 minute consultation.
